Decision boundary

The specification of which decisions a system may take unsupervised, which it may only recommend, and which stay with a person. Written as policy and implemented as code, so that the two cannot drift apart silently.

Common causes

Boundaries drift when the policy lives in a document and the enforcement lives in a prompt.

What good looks like

A policy version attached to every decision record, an enforcement service that refuses out-of-boundary actions, and a review that compares the two on a cadence.
