Reconciliation break
A discrepancy between an on-chain balance and the corresponding balance in an off-chain system of record, at a point where both are expected to agree. A break is a normal operational event; an undocumented break procedure is a control failure.
Common causes
Rounding rules that differ between systems; transactions counted before finality; a manual adjustment made in one ledger only.
What good looks like
A named owner, a stated tolerance, an escalation threshold, and a written procedure that says which record is authoritative while the break is open.
